1. Name of the post:-  Facility Level Quality manager(FLQM).

                  Qualification:- a)  MBBS/Dental/AYUSH/Nursing/Life Science/Social Science Graduate with Master in Hospital Administration / Health Management.

                                         b) Fluency in English, computer literacy, knowledge of legislation and policies are essential.

                                        c) Candidate must have good communication skill written and verbal.

                           Remuneration:- Rs  35,000/- per month.

                             Age:-   Upto 40 Years.

2.  Malaria Technical Supervisor(MTS):-

     Qualification & other Eligibilities:- 

 a) Graduate from Biology Stream or Graduate in any stream with science subject in class XII

b) The candidate should have a valid two-wheeler driving license.

c) The candidate should be  permanent resident of Jalpaiguri District.

Experiences:-

At least one (1)years working experience in a health related programme would be preferred.

Remuneration:- Rs  17,720/- per month.

                             Age:-   Upto 40 Years.

3. Lab-Technician:-

 Qualificaion:-

Passed Higher Secondary (10+2) Examination with Physics Chemistry and Biology/Mathematics and two years Diploma in Medical Laboratory Technology (DMLT) recognized  the Govt. of West Bengal State medical Faculty /AICTE.

Experience:- Not mentioned.

Remuneration:- Rs  17,720/- per month.

                             Age:-   Upto 40 Years.

 3. Self Attested Photo copy of Admit card of Madhyamik or equivalent examination. 

4. Self Attested Photo copies of Mark Sheets - Madhyamik & Higher Secondary Examinations. 

5. Self Attested Photo copies of All Mark sheets (Semester !Year wise) of Bachelor Degree, Master Degree and Management degree (as per post criteria) 

6. Self Attested Photo copy of Caste certificate (to whom applicable)

 7. Self Attested Photo copy of Diploma/Degree in Medical Laboratory Technology (DMLT) recognized by the Govt. of West Bengal State medical Faculty I AICTE as per post criteria.

 8. Self Attested Photo copy of Computer Course Certificate as per post criteria.

 9. Self Attested Photo copy of Experience Certificate (Experience certificates must consist of Name of the post, Employer's Name, Employee's Name, Date of Joining (DOJ) and Date of Leaving (DOL) otherwise experience certificates will be treated as invalid). 

10. Self Attested Photo copy of voter card I Aadhar card I other address proof.
